EJH Sports Pre-Season Checklist

1. 1) Current Physical- these are good for two years. You will need to make an appointment with your doctor.

3. 2) Concussion Screening- these are good for two years. You can make an appt. with OSS Physical Therapy. Appointments must be made online. Screening costs $15. You will need a concussion screening to participate in Football, Wrestling, Volleyball, Basketball or Soccer. *Stay tuned for specific dates for JH Fall athletes.*

5. 3) Electronic Participation Forms- these forms are located at the following website: puyallupschools.rankonesport.com (“Athletic Eligibility” and “Parent Permission and Consent”). Everyone needs to have these completed to participate. The forms can be accessed on August 1st, for the 21/22 school year.

7. 4) EJH Travel Card/Team Rules- these forms need to be filled out by a parent or guardian. The EJH team rules sheet needs to be signed by both the parent and student athlete.

9. 5) ASB/Transportation Fee- Athletes in the PSD are required to purchase a school ASB card ($30), and a transportation fee ($40 per sport) to participate. Athletes trying out for a “cut” sport can wait until after the team selection has been made to make their payments. These can be paid online, or in the office with Mrs. Kissner.

11. 6) Final Clearance- once all forms are completed in full and given to Mr.Dyer, each athlete will be issued a clearance card, to give to their coach on the first day of practice.
